Disability Evaluations and Documentation
Our Pain Medicine Practice is committed to providing comprehensive, evidence-based evaluation and treatment for patients with acute and chronic pain conditions. Our goal is to improve pain control, physical functioning, and overall quality of life.
Please note that our practice does not perform formal disability evaluations or provide disability determinations.
Our physicians do not conduct evaluations or complete certifications for Social Security disability benefits, permanent or temporary disability status, occupational disability, or formal work capacity determinations.
While chronic pain can significantly affect daily activities and employment, assessing eligibility for disability benefits or determining an individual's capacity to perform specific occupational duties generally requires an evaluation that extends beyond the scope of routine pain management services.
What We Can Provide : As part of your medical care, our clinicians document relevant diagnoses, examination findings, treatment history, responses to treatment, and clinically relevant functional information. These records may be requested for use in a disability application or for review by another qualified medical professional.
If You Need a Disability Evaluation: We encourage you to contact your primary care provider or an appropriate occupational medicine, rehabilitation, or disability evaluation specialist to discuss the requirements of your application.
If you are scheduling an appointment specifically to obtain disability certification or to have disability forms completed, please inform our scheduling staff before your visit so that we can clarify the services available.
Please be aware that scheduling a consultation with our practice does not constitute or guarantee a disability assessment, disability certification, or completion of disability-related paperwork.
We remain committed to helping patients manage their pain conditions and access appropriate medical care.
